By using the services offered on www.myhealthchecklab.com , you agree to be bound by these Terms & Conditions and other applicable laws. My Health Check Lab reserves the right to make changes at any time and changes will be effective immediately upon publishing. If you do not agree to comply with these Terms & Conditions, please do not use this website.
By utilizing this site, you agree to the following:
- you are over the age of 18;
- the information you submit while purchasing a lab test is truthful and accurate;
- you will not copy any images, text or structure from this website;
- you will use the site according to instructions and intended use; and
- you will abide by the Purchase Policy and Medical Disclaimer as defined below.

The information on this website is for informational purposes only. My Health Check Lab makes no expressed or implied warranties, representations or endorsements whatsoever with regard to the services provided. It is solely your responsibility to evaluate the accuracy, completeness and usefulness of all opinions, advice, services, merchandise and other information provided. My Health Check Lab shall not be liable for any cost or damage arising either directly or indirectly from any transactions conducted or information conveyed on the website. My Health Check Lab offers no assurance of uninterrupted service or error free service. My Health Check Lab does not warrant the accuracy or completeness of the information, text, graphics, links or other items contained on the website.
The users of this website agree to indemnify, defend and hold harmless My Health Check Lab, its officers, directors, employees, agents, licensor's, suppliers and any third party information providers to the website from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation of these Terms & Conditions by the user.
Discount coupon codes issued by My Health Check Lab are issued as a courtesy to our customers. Orders placed that do not meet the requirements or after expiration will be processed without the discount. Monthly specials will only be honored during the month they are featured. Site-wide percentage-off sales may exclude some tests. Health Care Providers are not eligible for additional discounts. Refunds will not be given retroactively. Coupons are not redeemable for cash and only one may be used for each purchase. Adjustments, credits and/or refunds for discount codes not entered at the time of ordering will not be issued. Payment is due at the time of service.
This website shall be governed by and construed in accordance with the laws of the State of Tennessee without giving effect to any principles or conflicts of law. This website is owned and operated by My Health Check Lab. All of the information contained herein is the exclusive property of My Health Check Lab.
Please note that My Health Check Lab only operates within the United States and due to state laws prohibiting direct-access testing, we are unable to offer services in New York, New Jersey, Rhode Island, Maryland, and Massachusetts.
Please also note that My Health Check Lab requires that patients tested be at least 18 years old. My Health Check Lab is unable to perform lab testing for anyone under the age of 18 at this time.

Service Satisfaction Guarantees
Only My Health Check Lab guarantees your satisfaction. If for some reason you are not completely satisfied or in agreement with the results of your test and want a redraw, email us at healthchecklaboratories@gmail.com or call 1-855-738-4832 within seven (7) business days of your original draw date.

There are numerous medical diseases that cannot be discovered by clinical lab tests alone. If a laboratory test result is outside normal range, you are encouraged to see a physician. If you do not have a physician we can provide you with a list of physicians in your area.

Laboratory test results may vary depending upon age, sex, time of day blood sample is taken, diet, medications, and the limits of modern technology. Many tests, in order to be validated, will need to be repeated at the discretion of the consulting or treating physician.
My Health Check Lab shall not be liable to you or anyone else for any loss or injury caused in whole or in part by procuring, compiling, interpreting, delivering or reporting information through this website. In no event shall My Health Check Lab be liable to you or anyone else for any decisions made or action(s) taken or not taken by you in reliance on such information.
If purchasing a blood test for the Human Immunodeficiency Virus (HIV) you understand and agree to the following:
The tests can produce false positive and false negative results. Screening tests fail to detect anti-HIV for a period of time immediately after infection with the virus (early post-exposure period). Re-testing or alternative DNA by PCR testing is a recommended option if it is suspected that post exposure testing has pre-dated the required period for conversion to positive HIV status.
The results of the HIV test will become part of your medical record. The confidentiality of your medical record will be maintained as per HIPAA regulations. However, it is required by law that a positive test results must be reported to the appropriate State Health Division.
